Replace a Lost Award
If you have lost, damaged, or never received a display award you earned from MCTC, you can request a replacement. Awards are printed off-campus and are only ordered three times each year, so you may not receive your replacement for several months after you make a request.
- Online Form: Submit a Replacement Award Request eForm
- Alternate format: Replacement award request form (PDF)
Only the name (or preferred name) currently on your record will be used on the replacement award.
Consider an official transcript
Your official transcript often is used to verify the credentials you have earned, so an official transcript may also meet your needs and can be provided more quickly than a replacement award.
Nursing Assistant/Trained Medical Assistant
Minneapolis College does not provide display awards for Nursing Assistant or Trained Medication Assistant.
If you completed NAHA 1819 after 2003, your transcript will note that you earned a Nursing Assistant Certificate. If you are being asked to verify that you completed a Nursing Assistant course, you will need to order an official transcript. To check if you are on the MN Nursing Assistant Registry, contact the Minnesota Department of Health.
If you are being asked to verify that you completed the Trained Medication Assistant course, you will need to order an official transcript.
