Registration Forms
- Course cap override (pdf) – after course waitlisting has ended for the registration period, use this form to request permission to be added to a full course. Visit the Wait Lists page for complete information about full courses.
- Late registration (pdf) – use this form to request registration after the deadline to add has passed. Visit the Late Registration page for complete information about late registration.
- Prerequisite verification (requires StarID login)– use this form to request registration when you have been blocked by perquisite screening. Visit the Prerequisite Screening page for what you need to know about prerequisite verification.
- Senior citizen registration (pdf) – MN residents who are at least 62 years of age may take courses for credit at a reduced rate, or audit courses for no charge.
Appeals and Exceptions
- Retroactive drop petition (MS form) – use this form if you did not intend to take a course, but the deadline to drop it has passed. Visit the Add, Drop, or Withdraw from Courses page to determine if this is an appropriate request for your circumstances.
- Exception to deadline – grade method or withdrawal (pdf) – use this form if you have a valid, documented reason for missing the withdraw deadline for your course or to request a change to the grading method for your course after the deadline has passed.
