Using the Blank Request Form
If you cannot locate an item you would like to request in the library catalog, you can fill out a blank request form. Complete as much information as possible, and in particular, complete every required field (labeled with a ‘*’), including a date for “not needed after”, and the copyright notice checkbox (for articles.) Select the link that best matches your request: Book, CD, DVD Request Form or Journal Article Request Form.
